Work Type: Full Time \ 40 Hours per week \ 8 am to 5 pm

Requirements: LCSW, LMSW-CC or LSW, LSW-C with 2 years of experience

Summary: Serves in an expanded clinical and care management role to collaborate with patients, Primary Care Providers, practice teams, and all community agencies, to provide care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Assesses patient needs from a strengths-based perspective and understands environmental factors that impact health outcomes. Designs and implements coordinated intervention plans that optimize patient’s self-management of his/her health and life in ways that are fulfilling. The Social Worker works collaboratively within the Integrated Care Team. The expertise of the Social Worker is sought to assess and intervene with bio-psycho-social-spiritual barriers and to enhance patient engagement with the healthcare system in a manner that promotes optimal health. Utilizes evidence based shared decision-making models, brief treatment models, motivational interviewing, and other evidence-based interventions to encourage positive behavior change in order to assist patients to achieve their highest level of function.
