Two Rivers Hospice is located in Carrollton, VA. We are a small start-up, locally-owned hospice that will provide care in Isle of Wight County communities and the surrounding area. We are committed to providing families with exceptional end-of-life care.

Duties: What you will do:

• Provide support to patients and families
• Assist patients and families in obtaining needed services
• Conduct initial psychosocial assessments of new patients
• Travel to patients wherever they reside: Home, nursing facility etc.

Requirements: What you need to have:

• Master of Social Work (MSW)
• Valid social work license
• Driver’s license, insurance and vehicle
• 1-2 years of healthcare experience (hospice preferred)
• Utmost compassion and integrity

Works in a team approach with other qualified professionals, as determined by the hospice, to evaluate patient/family response to care

Participated in IDG conference when the plan of care is reviewed

Must comply with company policies, procedures, and processes in addition to Hospice rules and regulations

Must comply with harassment and discrimination policies

Must be dependable and flexible in work schedule
