Origins has positioned itself as a trusted partner in the cannabis industry for individuals who want to experience premium medical services. We are in the Downtown Salt Lake/South Salt Lake area.

We are looking for an APRN/NP or MD/DO to become a registered QMP (Qualified Medical Professional) to assist those patients who qualify under Utah’s law to help with medical cannabis cards.

Scope of Work Includes:
-Review outside medical records for qualifying medical conditions
-Document evaluations and recommendations in accordance with all regulatory agencies
-Input patient information into the state website (EVS) and assist patients with their end of EVS
-Answering patient questions and assisting with resources (make referrals to alternative modalities as deemed necessary)
-Stay up-to-date and follow Utah’s guidelines regarding medical cannabis

Qualifications:
-Must have an active, unrestricted license and active DEA
-Your license must be in good standing

Ability to commute/relocate
-Reliably commute or plan to relocate before starting work

Preferred
-Already registered in Utah’s QMP program (if not, the Company will compensate for training and certifications if needed)

Pay: $100.00 - $200.00 per hour
