Nurse Practitioner. Hospice experience preferred.
$55/hr. Pay rate negotiable based on experience.

The Nurse Practitioner is responsible for:

• Providing clinical expertise including face to face visit encounters with the patient.
• Working with the Medical Director and collaborating with other physicians and non-physician providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological and spiritual needs of the patient.
• Providing expertise in a consultative role to patients with advanced disease requiring palliative services.
• Providing physical back up and coverage responsibilities where appropriate including writing prescriptions.
• Shares Hospice on-call responsibilities with Medical Director
• Performs other duties as assigned by the Medical Director and Clinical Director
• Assisting in the development and review of clinical protocols, policies and processes that support appropriate referrals and care
