Who we are?
Bloomer Health is a therapist focused Early Intervention agency, that provides services to babies and toddlers aged zero to three years old. Our agency uses an innovative, digital platform built by therapists, for therapists, to modernize and reduce time spent on non-clinical tasks (e.g. onboarding, case management, documentation and billing). We actively support the therapist from behind the scenes to ensure smooth delivery of services and, where, possible, help save time on case management. Our platform is further integrated into billing which makes it easy to process, no work for the therapist and allows us to pay you quicker!
Provider's Responsibilities
The Physical Therapist (PT) will be working 1 on 1 with developmentally delayed babies and toddlers who qualify for Early Intervention services. Utilize their clinical knowledge and skills to help the child reach their gross motor milestone goals. Sessions are prescribed from 1 to 5 times per week for 30 or 60 minutes. They educate and guide parents and caregivers to ensure carryover between sessions.
At Bloomer Health, we support and mentor providers, by being responsive to each therapists’ unique needs. We also offer around the clock access to our administrative and technical support teams. We have clinical directors for each discipline, who, via a strong quality improvement process, are able to provide clinical support to each therapist. You will never feel alone.
Flexibility
Therapists are able to create their own schedule and workload based on personal availability and preferences. You will be a part of a care team that will include the family, care coordinators, other service providers, and Bloomer Health support staff.
What qualifies a provider?
· A degree in Physical Therapy (PT) from an accredited university: MPT, DPT
· Active Physical Therapy (PT) license with the NYS Office of Professions
· At least one (1) year of experience working with children aged 0-5 years and their families
· DOH Approval is great, but not a must
· Ability to independently set and manage a caseload, process documentation in a timely manner and effectively communicate with families and Bloomer Health personnel
