Cloverleaf Health Care Center is hiring a Physical Therapist and Physical Therapist Assistant to provide rehabilitation services in one or more of our service locations in Knightsville, IN

The Physical Therapist / Physical Therapist Assistant is responsible for assessing the need for, developing, and delivering physical therapy programs to facilitate rehabilitation.

Shift Available

• PRN Physical Therapist Assistant

• 20 hours will be split at Oak Village Healthcare in Oaktown, IN and at Cloverleaf Healthcare Center in Kightsville, IN

$7,500 Sign on Bonus

Responsibilities:

• To provide quality physical therapy care within the PT scope of practice in accordance with established standards of practice, department procedures and productivity standards
• To assist in the evaluation process and with identifying a target list, including short-term goals, long-term goals and treatment plan

• Licensed Physical Therapist; BLS certification
• Covid Vaccine required unless an approved exemption is presented upon hiring
