Job Summary

The Population Health Specialist will be responsible for collaborating with the population health team on identified strategies for the Sanford Health Plan membership preventive care and health outcomes. Helps to develop strategies and deliver on assigned population health work. Works collaboratively with the Manager, Population Health on identified strategies for the department. Contacts and interacts with members, which may include preventive care education, coaching, and connecting members to beneficial resources. Helps to develop strategies, which address the social determinants of health needs of identified membership populations. Researches current and new resources, ideas, plans, and strategies. Assists members to achieve their individual health goals as well as recognizing the overall health goals and progress of the population. Is flexible to changing strategies, based on the medical economic trends identified in Sanford Health Plan(SHP) membership. Evaluates success of current strategies, which may include data collection and evaluation and/or collaboration with other SHP or HSD departments. Provides insight and suggestions on population health interventions based on best practice and evidence-based research. Assists in implementing population health initiatives, including planning, collaboration, data gathering, teaching, coaching, and/or reporting. Possesses and seeks comprehensive knowledge and understanding of population health standards/methodologies, as well as innovation.

Exhibits strong verbal and written communication skills, attention to detail, commitment to organization and department values. Able to work autonomously, takes initiative, and can lead departmental programs. Demonstrates problem-solving skills to analyze issues and create action plans, can facilitate small group discussion and speak publicly.

Qualifications

Bachelor's degree in healthcare related field required; master's degree in public health or related healthcare area is preferred.

1-3 years experience in a healthcare or health plan setting is preferred. Experience with clinical improvement, project management, and/or member outreach beneficial. Knowledge of quality measures (HEDIS, NCQA, etc.) beneficial.
