You must be located in Milwaukee WI or no more than 20 miles outside of Milwaukee. This job does require home visits, upon the expiration of the city ordinance as it relates to Covid19. We are temporary completing virtual visits (zoom, phone, email etc). Some services may require you to conduct a face to face visit (this will vary).

Job functions.......Be fluent in English and Spanish both written and verbal. Perform initial intake assessments when enrolling pregnant and/or non pregnant clients who receive state benefits in the county of Milwaukee. Provide resources, advocate, follow up and create achievable goals. Ongoing care coordination. Attend and assist with group and individual health and nutritional training. We are currently working remotely due to Covid19 but some duties may require you to come into the office. Must reside in Milwaukee WI.
