This is a professional position responsible for the provision of a wide range of psychosocial services to our residents and their families. The services provided may include: supportive counseling, problem solving, community referrals, financial education and guidance.

Essential job functions

• Function as a support person for the patient/family/caregiver on an ongoing basis.
• Function as an expert and facilitator at obtaining and making referrals to community resources to meet the needs of patients and families and obtaining financial assistance when indicated.
• Coordinate resources for respite care needs.
• Provide and maintain a safe environment for the patient.
• Assist patient/family/caregiver and other team members in providing continuity of care.
• Demonstrates an ability to prioritize work responsibilities effectively.
• Demonstrates ability to organize, manage and function in assigned areas, completing work in a timely and organized manner.
• Maintains confidentiality of patient and agency information according to HIPAA guidelines with regard to patient information.
• Ability to develop and maintain good working relationships within the community by maintaining existing resources and developing new ones.
• Able to coordinate multiple priorities and adapt to a variety of projects/tasks with flexibility.
• Deals with conflict situations in a professional manner

Requirements:

Minimum of 1-2 years of SSD experience in Long Term Care or

Licensed Social Worker degree from a school of social work accredited by the Council on Social work (Preferred but not required)
