POSITION SUMMARY

The Community Education Coordinator is responsible for the planning, coordination, and implementation of educational strategies and content aimed at increasing understanding and utilization of hospice, palliative care, and home health in and around the communities served by Adoray Home Health & Hospice. This includes the development and presentation of community and clinical education including in-services, continuing education, and competency education for Adoray referral partners. Represents Adoray in a professional manner, is passionate about end-of-life care, and exemplifies Adoray’s core values in daily practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Aligns short and long-term educational program goals and design with the strategic initiatives of the organization.
  • Responsible for census development through the implementation of external and internal education and sales programs with both existing and new clients.
  • Contact key providers and other healthcare representatives on a regular basis to establish a positive referral relationship.
  • Participate in short and long-range planning for the agency and implements specific measures for agency growth.
  • Increase market share through education of providers and other community and referral sources.
  • Work with other agencies and promotes good community relations through involvement in community events, health fairs, and professional/community groups.
  • Provides in-service education to referral facilities as requested and required by partner contract agreements.
  • Oversees the planning and implementation of community and facility educational programs (LTCF family education, Consider the Conversation, Benefits of Hospice, etc.) regularly.
  • Collaborates with educational institutions and students to provide meaningful educational experiences for students.
  • Assist in conducting market and community needs assessments to determine gaps in community education and understanding of hospice and palliative care.
  • Maintains professional contacts through staff development peers, and state and local organizations as applicable.
  • Monitors physician recruitment at local clinics and establishes and provides information to new providers within a reasonable time of hire.
  • Receives ongoing input from partner facilities on how to improve/change processes to ensure patient needs are being met.
  • Other duties as assigned.

Minimum Qualifications

  • Registered Nurse
  • Minimum of one-year clinical end of life or hospice experience required.
  • Current driver's license from state of residence.

Knowledge, Skills and Abilities

  • Display and promote high standards of ethical conduct and behaviors consistent with organizational standards
  • Exhibit excellent communication and organizational skills
  • Working knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook).
  • Ability to manage conflicting priorities and handle multiple tasks/projects concurrently
  • Ability to consistently carry out job responsibilities, able to acknowledge when commitments are not met, and take appropriate steps to fulfill responsibility
  • Ability to clearly and effectively convey information to individuals or groups, including ability to choose the appropriate method or methods of communication
  • Ability to analyze, identify, and solve problems while using sound judgment
