The Certified Diabetes Educator (CDE) is a nurse or dietitian who has a minimum of 1000 hours of professional practice experience in diabetes self-management training, and has passed the Certification Examination for Diabetes Educators.

• The CDE will implement clinical and education programs, be responsible for quality improvement and research activates, and be an expert resource for both patients and providers.
• The CDE may also be responsible for the ADA Recognized Program process.

• Provides consultation to inpatients, clinical patients, community residents and providers, facilitates communication between patients and providers and makes referrals to appropriate healthcare providers and community outreach
• Provides 1:1 consultation within the clinic setting as needed in to cover department demands
• This position is an outpatient role located at St. Anthony's Hospital
