The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides physical therapy services to patients accepted for care by the RVNA as prescribed by the attending physician. The therapist is responsible to the Director of Rehabilitative Services for fulfilling the needs of the agency in the area of his/her expertise.

We have 2 full time positions available, one covering the Danbury, CT area and the other covering the New Milford, CT area!!

Job Responsibilities:

• Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan with RN case manager, patient, family and physician.
• Identification of patient and family needs for other home health services and referral for same when needed.
• Participation in case conferences.
• Instruction of patient, family and other agency health care personnel in the patient's treatment regime when indicated.
• Supervision of Home Health Aides/Homemaker s when such personnel are participating in the patient's therapy regime.
• Assists patients to reach a level of function to enable them to remain safely in their home environment.
• Obtains written physician's order for Physical Therapy every sixty (60) days, more often if necessary.
• Reports in writing the patient's condition to the physician at least every sixty (60) days.
• Maintains clinical progress notes which include continuing assessment treatment (s) results noted and communication with other personnel.

Experience & Qualifications:

• Graduate with a baccalaureate degree of an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education with collaboration with the American Physical Therapy Association.
• Current license to practice as a Physical Therapist in Connecticut.
• At least two (2) years' clinical experience.
• Knowledge of physical therapy methods with particular skill in casework in the home setting.
• Ability to adapt this knowledge and skill to a home setting.
• Knowledge and understanding of the principles of home care and community health administration and practice in the home setting.
