At Home Harmony is transforming how healthcare is delivered to aging seniors and medically vulnerable members in our community. Our private practice is focused on keeping seniors healthy by providing care in their homes through an interdisciplinary team of providers, pharmacists, nurses, social workers and others – all working together to meet our patients’ needs.
We are seeking an experienced and talented Nurse Practitioner / Physician Assistant to join our team. The NP/PA will diagnose and treat health conditions, as well as promote good health practices for aging seniors that we serve in the home. The successful candidate must be able to work independently, as the NP/PA will be responsible for the successful management and care coordination of an assigned patient population. There is always a physician available for consultation.

Job duties include but are not limited to:
· Work as part of a clinical care team to deliver care in the patient’s home which includes driving or riding in a vehicle to the patient’s home or residence.
· Perform comprehensive health assessments; diagnose and treat complex illnesses.
· Perform Medicare Annual Wellness Visits and follow Chronic Care Management practices in patient care/documentation.
· Operate at the center of a coordinated care model that ties together our in-home care, along with pharmacy services, remote monitoring and other critical services to keep patients well and thriving in their homes (and out of nursing homes)
· Use critical thinking skills and follow evidence-based standards of care.
· Maintain accurate records and schedules.
· Analyze test data to diagnose, treat and arrange appropriate follow up for the patient and family members to continue seamless care coordination.
· Cautiously and compassionately navigate difficult conversations related to end-of-life issues and goals of care.
· Communicate effectively with all individuals involved in the patient’s care.
The position is structured as follows:
· Full-time (preferred) or part-time

Experience and Qualifications:
· Minimum five years’ experience
· Current Virginia NP or PA License, required
· Primary care and geriatric experience, preferred
· Self-motivated, directed, flexible and a team player
· Positive attitude and customer service orientation
