POSITION TITLE: Access Liaison Nurse
REPORTS TO: Access Team Supervisor
FLSA: Exempt
Position Scope:
The Access Liaison Nurse, under the direction of the , Access Team Supervisor, utilizes effective customer service skills and social marketing strategies to build relationships with territory referral sources in a results-oriented manner. Sets and achieves goals in regard to Access Team goals for referrals, admissions, length of stay, and census.

I. Essential Functions/Performance Factors
o Identify and profile key local hospital professional referral sources.
o Maintain existing relationships and build news relationships as new referral sources are identified.
o Utilize reports from Valley Hospice data base, track and trend referral patterns from key hospital providers.
o Participates in agency sponsored events as indicated, e.g. volunteer recognition, bereavement activities, fundraising events, community events, e.g. Relay for Life, Heart Walk or other events as requested.
o Work with Access Team Manager to determine frequency of calls on key hospital referral sources.
o Consult with hospital referral sources to assist them in determining hospice eligibility.
o Work with Access Team Manager to set specific goals for referrals from hospitals.
o Design and employ ethical and effective strategies for meeting referral goals.
o Work with hospitals to identify patients who would benefit from hospice and/or palliative care.
o Meet with patients and families to explain hospice services and payment.
o Get consents for hospice care signed.
o Facilitates transition to home care in conjunction with hospital discharge planners, as indicated.
o Manage discomfort and provide symptom relief.
o Utilize specialized nursing skills related to palliative and end-of-life care.
o Assess patient’s physical condition with each contact and document findings.
o Complete a thorough pain assessment, when pain is identified as a problem, identify immediate needs related to pain, and implement appropriate interventions.
o Identify other symptoms, determine immediate needs related to these symptoms and implement appropriate interventions.
o Evaluate the effectiveness of nursing interventions and make appropriate changes to the care plan.
· Facilitates admissions to general inpatient (GIP) level of care under the Medicare Hospice Benefit and other payors; coordinates and manages care for these patients while hospitalized, as needed.
o Work with hospitals to identify patients who would benefit from hospice and/or palliative care.
o Meet with patients and families to explain hospice services and payment.
o Complete paper work associated with direct admissions from acute care to the hospice general inpatient level of care.
o Use the nursing process to assess the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living and identify immediate needs of those patients who utilize the on-call line.
o Provide education, instruction and support to patients, families and caregivers who utilize the Valley Hospice services.
o Identify the need for nursing, psychosocial, and spiritual visits.
o Inform the physician and other personnel of changes in the patient’s needs and outcomes of interventions.
o Assist with the smooth transfer of patients to other levels of care and facilities as appropriate.
o Effectively communicate outcomes of calls to appropriate members of the appropriate hospice teams.
o Facilitates transfer of patient to the Care Center, as indicated.
o Complete all admission paperwork accurately.
o Write nursing notes that reflect the patient’s terminal illness according to NHPCO guidelines, Local Medical Review Policies and the care plan.
o Sign all entries with full signature & credentials.
o Research and prepare up to date presentations annually to contracted hospital staff.
o Provide individual consultation regarding palliative/hospice care, as indicated.
o Check with all units regularly to obtain information regarding referrals.

Certifications and Licenses:
RN, WV and OH
Certified Hospice & Palliative Care Nurse (CHPN) after two years of hospice experience
Current driver’s license, automobile insurance, and reliable transportation

WORK ENVIRONMENT
While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites. Patients’ home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment in a controlled atmosphere building.
